information and to submit the necessary forms for their student
athletes. This method will save you time and allow you to have
the necessary documents available for your current and future needs.
Please
follow the directions below to download and view important
athletic-related data
for your child. Always feel free to

Mogadore Local Schools is required to provide this information to you by the
Ohio
High School Athletic Association and review of this information by
you and your child is a prerequisite for participation on our
sports teams grade 7 to 12. |

Directions -- All Steps Must Be Completed
- Download
and print the "Parent Information and Forms Sign Off Sheet".
Complete all data at the top and bottom of the form.
- Review
the "Let's
Get it Started"
video from the OHSAA. This video is 20 minutes in length and covers
many important topics.
- Download
and review the "Extracurricular Activity Code of Ethics for Mogadore High School".
This brochure will provide a complete overview of the behavior
expectations for our student athletes. It also contains our
Alcohol, Tobbacco and Drugs policy.
- Download
and review all 3 sheets published by the Ohio High School Athletic Association (OHSAA) "Eligibility Checklist" "Eligibility
Brochure" & "Eligibility
Guide".
- Download
and review the OHSAA "Sportsmanship for Parents". This is a two page document.
- Download, print and then complete "Emergency Medical Authorization"
- Download
and review the "Mogadore Athletic Booster Club Brochure and Membership Form" *.
- Once
you have reviewed all documents, keep all items for yourself, both the parent/guardian and student athlete must sign and
return the "Parent Information and Forms Sign-off Sheet" (#1 above) with your child
to his/her sport coach. Mogadore student-athletes must have this completed form turned in to their respective coach no later
than the first official day of practice.

| 1. Preparticipation Physical Evaluation | You will need to print this and complete a portion before
you go for your sports physical. Be sure to have the attending
physician include their office stamp and signature on the back. |
