SPORTS PHYSICALS FOR ALL MOGADORE ATHLETES Dear Parent/Guardian Mogadore athletes entering grades 7-12 have the opportunity to get their sports physicals at the Children’s Sports Medicine Center on Tuesday, July 29, 2008 from 5:15 p.m. - 7:15 p.m.  The cost will be $15.  In order to accommodate your schedules better and to expedite physical processing, we are holding this event BY APPOINTMENT ONLY at the Sports Medicine Center, Akron campus (address below). All appointments will be made through Sports Medicine Center. You will find the official OHSAA physical forms that are REQUIRED to be on file prior to your child’s participation in any Mogadore JR/SR High school athletic function at this website: http://www.ohsaa.org/medicine/PPE.pdf.  Please print this form, complete the front page, sign it, and bring it to your scheduled appointment. The forms are also available in the high school office. It is best if parents are able to accompany the child to his/her physical appointment in order to answer questions and to be present for any new concerns.  Otherwise, parents and athletes MUST fill out the front of the form, as well as sign it and the attached second page BEFORE the physical appointment.  Please note that there are three places you and your child are required to sign!  If you plan on getting a physical somewhere else, you must still use this form.  In order to schedule your appointment, please call the Sports Medicine office at (330) 543-8260.  You must state specifically that you are scheduling for Mogadore’s physicals or you will be charged the clinic’s regular physical fee of $30.
